Forms
After forms are set up in axiUm, you can modify elements of a patient form in the following ways:
For patient forms, you can hide:
- Question numbers.
- The Form code on the iForm List page.
- The date field on the iForm list page.
To hide elements for patient forms:
- On the Settings tab, click Forms.
- In Parameters, select 1 in one of the UI* boxes, and then enter 0.
- Click the Save button.
You can customize text on form controls and customize instructions related to updating answers.
To customize form controls or patient instructions:
- On the Settings tab, click Forms.
- In Parameters, Navigation Controls or Labels/Messages, select one of the boxes, and edit as required.
- Click the Save button.
Instructions are shown when a patient initially opens a form. You can configure instructions, so that they always show when a patient opens a form.
To show instructions for a form by default:
- On the Settings tab, click Forms.
- In Parameters, select 0 in the UI.ExpandFormInstructionByDefault box, and then enter 1.
- Click the Save button.

To show question instructions for a form question by default:
- On the Settings tab, click Forms.
- In Parameters, select 0 in the UI.ExpandQuestionInstructionByDefault box, and then enter 1.
- Click the Save button.
